NHS England publishes quarterly performance data on cancelled elective operations, which includes national, regional and provider level activity, and this can be accessed at:

https://www.england.nhs.uk/statistics/statistical-work-areas/cancelled-elective-operations/cancelled-ops-data/

NHS Digital published a report for 2016-17 surrounding Hospital Outpatient Activity which incorporates cancellation data, including national, regional and provider level activity, and this can be accessed at:

http://digital.nhs.uk/catalogue/PUB30154

Information for postponed appointments is not collected centrally.
