The Government should make the MHRA’s Yellow Card reporting system mandatory for medical professionals to complete whenever they identify or are made aware of an adverse reaction.
All yellow card reports must also be sent to the responsible pharmaceutical company immediately.
This will ensure that medical professionals have a responsibility to report, with appropriate sanctions if they fail to do so. It should increase reporting and help improve patient safety and confidence.
