A public inquiry into the entire NHS maternity services to identify the root causes of failures and implement effective solutions to prevent further harm. This inquiry must encompass all aspects of maternity care, including funding, staffing, training, infrastructure and resources.

Every mother and baby has the right to receive high-quality, safe care; we believe it is evident from recent inquiries that the current system is failing to deliver on this fundamental right. To ensure transparency and credibility and to restore public faith this inquiry needs to be accessible to the public, ensuring accountability and commitment. We believe that the greatest catastrophe for maternity services would be to treat the reviews of Ockenden, Kirkup et al as isolated incidences.
