Review standards of clinical/care home record keeping

Review whether NHS record keeping is being carried out at the time of treatment/care and not afterwards. In a legal challenge the NHS or care home records may be accepted as truth over any recorded observations/evidence by families.

We think allowing the NHS or care homes to complete records after treating patients or residents in care - that should be completed during treatment - can mean that the accuracy of the record becomes distorted and allows bad practice to be built into the system. Removing bad practice would build trust with patients/loved ones. This has been criticised in an official review and we feel it has not been addressed.
