Refocus investigation of maternity care and expand to include all NHS trusts

Families have suffered birth trauma, injuries, and loss due to unsafe staffing and overstretched maternity units. Many NHS trusts are not part of current investigations. We think a full national investigation and increased funding for midwife recruitment and training are urgently needed to ensure safety and accountability for all families.
