To ask the Secretary of State for Transport, whether he has responded to the Regulation 28 report issued by Andre Rebello, Senior Coroner, Liverpool and Wirral area, on 1 September 2015, in relation to the death of Stephen O'Malley; and if he will make a statement.

The Marine Accident Investigation Branch of the Department for Transport has received a copy of a Regulation 28 Report from the Senior Coroner for Liverpool and Wirral Coroner Area. The Report was addressed to SubCPartner, a Danish company based in Esbjerg. Given the circumstances of Mr O’Malley’s tragic death on 3 May 2012, the Chief Inspector of Marine Accidents is sharing the Report with his counterpart at the UK Health and Safety Executive, which is the UK authority responsible for the investigation of diving accidents, so that lessons may be learned and promulgated with the aim of preventing any such recurrence.
