Personal Independence Payment

Question to the Department for Work and Pensions:
To ask the Secretary of State for Work and Pensions, how many and what proportion of personal independence payment decisions have been overturned at (a) the mandatory reconsideration, (b) the appeal and (c) any other stage as a result of information being incorrectly recorded during face-to-face consultations.
