Question to the Home Office:

To ask the Secretary of State for the Home Department, if she will discuss with the (a) Attorney General, (b) Crown Prosecution Service, (c) Secretary of State for Work and Pensions and (d) Health and Safety Executive the reasons for the time taken by the Independent Police Complaints Commission to conclude its further investigation into the death of Thomas Orchard on 3 October 2012; and if she will make a statement.
