Question to the Department of Health and Social Care:
To ask Her Majesty’s Government what assessment they have made of whether the HSA4 abortion notification form for practitioners terminating a pregnancy remains up to date.

The Department keeps the content of the HSA4 abortion notification form under review. There are currently no plans to update the form.
