Question to the Department of Health and Social Care:
To ask Her Majesty’s Government whether they have plans to update the HSA4 abortion notification form for practitioners terminating a pregnancy; and if so when, and in what way.

The Department keeps the content of the HSA4 abortion notification form under review. There are currently no plans to update the form.
