Question to the Department of Health and Social Care:

To ask the Secretary of State for Health, how many never events have occurred involving (a) audiologists, (b) cardiac physiologists, (c) gastro-intestinal physiologists, (d) neurophysiologists, (e) respiratory physiologists and (f) sleep physiologists in the last five years.

This data is not collected centrally. Never events are reported through a tool called the Strategic Executive Information System which does not routinely collect information on the specialty of staff involved in never events.
