Question to the Ministry of Justice:

To ask the Secretary of State for Justice, what estimate he has made of the number of prevention of future death notices issued to each NHS ambulance trust in each of the last five years.

The Ministry of Justice does not hold this information. Since 1 April 2013 coroners have been required to send a copy of all their reports to prevent future deaths to the Chief Coroner. In most cases the Chief Coroner will publish the reports and responses on his website at the following address although he is not obliged to do so: https://www.judiciary.uk/related-offices-and-bodies/office-chief-coroner/pfd-reports/
