Question to the Ministry of Justice:
To ask the Secretary of State for Justice, how many Prevention of Future Death Notices issued by coroners in England related to Accident & Emergency in each of the last five years.

The Ministry of Justice does not hold this information. Since 1 April 2013 coroners have been required to send a copy of all their reports to prevent future deaths to the Chief Coroner. In most cases the Chief Coroner will publish the reports and responses on his website at the following address although he is not obliged to do so: https://www.judiciary.uk/related-offices-and-bodies/office-chief-coroner/pfd-reports/
